  1. Collect three months of original bills that indicate your new SHOWTIME subscription from your service provider, dated between 1/1/15 and 3/31/16.

  2. Get your Rebate Form: Complete the entry fields below and select the Print button. Writing your name, address, and phone number on a 3" x 5" card also qualifies as a Rebate Form.

  3. Mail your three months of original bills and Rebate Form to the address below. Documents must be received by 4/28/16 to be eligible:

*If you signed up for a new Showtime subscription by 12/31/2015, through a cable, satellite or telephone company, you can get $25 cash back after three (3) months of paid service. All qualified $25 claims will be paid in the form of a $25 Visa® PrePaid Card.
